Step Two: Visit your Primary Care Physician and complete an annual wellness visit at no cost.

Step Three: Within 30 days of the claim being processed, the Trust will send you an electronic gift card for $100!

Telemedicine through Teladoc

Gym Memberships

The Trust partners with Active&Fit Direct so members can access premier fitness facilities and streaming workouts at a fraction of the cost. Memberships to standard facilities are available for $28 per person per month and premium facilities are available to members at a 20% - 70% discount off retail price. Both memberships come with additional streaming fitness content. To register for Active&Fit Direct, login to your Meritain account and click on “Messages and Links”.

  1. Members must enroll via www.aceclifehealthtrust.com/select-members prior to receiving the annual wellness visit to be eligible to receive $100 gift card.

  2. Applies only to members and partners / spouses. Child dependents are not eligible to particpate.

  3. Access to Noom is provided based upon continuous enrollment in the Select Plan.

  4. Members are entitled to a subsidized fitness tracking device once every 24 months.

  5. For the purposes of ERISA, this is not a healthcare benefits but a wellness benefit not subject to 5500 reporting requirements. These services are not being furnished by a medical professional.

  6. Employers are the plan sponsor, not the ACEC Life/Health Trust or Meritain Health.
