Employees and spouses that are enrolled in the Select medical plan administered by Meritain are eligible to earn a $100 incentive for completing an annual wellness visit with a physician from January 1 - December 31, 2024. To earn the incentive, the following steps must be completed:

Complete the below registration form to sign up for a $100 wellness incentive.
Schedule a visit with a primary care provider and complete your annual wellness visit. Provide your Meritain insurance information during the visit.
Once your annual wellness visit claim has been processed, you will receive a $100 gift card from ACEC Life/Health Trust
